JAMA -- Abstract: Practical Clinical Trials: Increasing the Value of Clinical Research for Decision Making in Clinical and Health Policy, September 24, 2003, Tunis et al. 290 (12): 1624 ()

„The widespread gaps in evidence-based knowledge suggest that systematic flaws exist in the production of scientific evidence, in part because there is no consistent effort to conduct clinical trials designed to meet the needs of decision makers. Clinical trials for which the hypothesis and study design are developed specifically to answer the questions faced by decision makers are called pragmatic or practical clinical trials (PCTs). The characteristic features of PCTs are that they (1) select clinically relevant alternative interventions to compare, (2) include a diverse population of study participants, (3) recruit participants from heterogeneous practice settings, and (4) collect data on a broad range of health outcomes. The supply of PCTs is limited primarily because the major funders of clinical research, the National Institutes of Health and the medical products industry, do not focus on supporting such trials.“

JAMA -- Abstract: Caffeine and ventricular arrhythmias. An electrophysiological approach, November 7, 1990, Chelsky et al. 264 (17): 2236 JAMA -- Abstract: Caffeine and ventricular arrhythmias. An electrophysiological approach, November 7, 1990, Chelsky et al. 264 (17): 2236

„Little information is known regarding caffeine's effect on the substrate supporting sustained ventricular arrhythmias. This prospective study evaluated the effect of coffee (275 mg of caffeine) on this substrate with programmed ventricular stimulation in 22 patients with a history of symptomatic nonsustained ventricular tachycardia, ventricular tachycardia, or ventricular fibrillation. Patients underwent electrophysiological testing before and 1 hour after coffee ingestion. Mean (+/- SEM) plasma caffeine level achieved after coffee consumption was 6.2 +/- 0.5 mg/L.“

JAMA -- Abstract: Comfort care for terminally ill patients. The appropriate use of nutrition and hydration, October 26, 1994, McCann et al. 272 (16): 1263 JAMA -- Abstract: Comfort care for terminally ill patients. The appropriate use of nutrition and hydration, October 26, 1994, McCann et al. 272 (16): 1263

„PARTICIPANTS--Mentally aware, competent patients with terminal illnesses monitored from time of admission to time of death while residing in the comfort care unit. MAIN OUTCOME MEASURES--Symptoms of hunger, thirst, and dry mouth were recorded, and the amounts and types of food and fluids necessary to relieve these symptoms were documented. The subjective level of comfort was assessed longitudinally in all patients. RESULTS--Of the 32 patients monitored during the 12 months of study, 20 patients (63%) never experienced any hunger, while 11 patients (34%) had symptoms only initially.“

JAMA -- Abstract: Neurosyphilis and penicillin levels in cerebrospinal fluid, November 8, 1976, Mohr et al. 236 (19): 2208 JAMA -- Abstract: Neurosyphilis and penicillin levels in cerebrospinal fluid, November 8, 1976, Mohr et al. 236 (19): 2208

„During intravenous and after intramuscular penicillin therapy, a spinal tap was performed on all subjects; later, assays were done. Of two patients who received intravenous penicillin G, one had 0.3 mug/ml and the other had 2.4 mug/ml penicillin in the CSF. Twelve of 13 patients who received benzathine penicillin G had no detectable penicillin in the CSF; one patient had 0.1 mug/ml penicillin in the CSF.“
